Does a data table have to be on the same tab?
The data table must be located on the same tab as the setup data table (as mentionned above in the Set Up section). You cannot link the data table to cells on a different tab.
Do data table inputs need to be on the same sheet?
Input cells must be on the same sheet as the data table unless you create clone cells. The formula cell can be on another sheet, but it must be fed directly or indirectly by the input cells.

Tables should be:
Labeled with the table number and descriptive title above the table. Labeled with column and/or row labels that describe the data, including units of measurement. Set apart from the text itself; text does not flow around the table.
